Enhanced Care Management

Enhanced Care Management (ECM) is a specialized program designed to assist patients with complex needs in managing their health and well-being more effectively. This program is focused on providing holistic care beyond traditional medical services, addressing both clinical and non-clinical factors that impact health outcomes.

Connects patients with vital resources
Provides personalized communication-Patients can choose their preferred method of communication, whether in-person, by phone, text, or email

Our Case Managers meet ECM members where they are. This may mean on the street, in a shelter, in their provider’s office, or at home. Through this program, staff will connect members to community support services that can help patients get access to nutritious food, safe housing, public subsidies, or whatever services are required to complement the care they receive by Peach Tree Health clinics. ECM focuses on special patient populations and helps select Medi-Cal patients avoid the challenge of engaging with multiple delivery systems for care and services.
